It would seem  to be counterintuitive for a plastic surgeon to write an article on how to avoid a face lift.  Frankly we probably know  more about the subject than anybody else since we've watched our patients literally  grow up and grow older in front of our eyes.   Not only are we trained observers, but we also have the knowledge and tools to slow down (and in some cases reverse) the aging process. A <a href="http://www.nbcnews.com/id/33385839/ns/health-skin_and_beauty/t/twin-study-reveals-secrets-looking-younger/#.VMvUENLF_To" rel="nofollow">recent study</a> performed by a plastic surgeon who made a point to visit an identical twin convention in Twinville, Ohio,  revealed a great deal of information concerning non-genetic factors contributing to the aging process.  First of all you can not deny the influence of genetics on the aging process.  We all have  known  individuals that appear to be much younger than others the same age that take care of themselves in approximately the same manner. There are factors, however,  that will make <b><i>even identical twins </i></b>look significantly different by the time they reach their 40s 50s and 60s. If you are young enough to learn from these twins mistakes then read on.<br />

Environment Overriding Genetics!</h3>

1)<i><b> sun exposure</b></i>: there is no doubt that sun worshiping or unprotected exposure during outdoor activities will significantly affect how old you appear.<br />

3) <b><i>prolonged use of birth control pills and hormone replacement therapy </i></b>actually had a very positive effect on protection from aging.  The controversies about and prolonged use of birth control pills notwithstanding<br />

4) <b><i>stress or the avoidance of stress</i></b> has a significant role in how you look. The divorced women that participated appeared to be 1.7 years older than they are married  identical twins! Paradoxically, widows looked two years the younger than they are still married twins.  While marriage can be stressful apparently divorce and financial difficulties can impact negatively on our aging process.<br />
<br />
5) <b><i>smoking</i></b> has long been known to affect the aging appearance.  The classic lines around the lips known as "smokers lines" as well as increase in generalized wrinkling and loss of elasticity are related to the direct negative effect on collagen elastin of tar and nicotine.<br />
<br />
6)  <b><i>excessive alcohol consumption</i></b> speeds up aging despite the current believes that her an ingredient in Red wine known as Resveretrol.  While a glass of wine may be good for your heart, too much can affect the liver and this will have a detrimental effect on the blood vessels in your skin as well as direct damage to your collagen.<br />
<br />
While these results give us information on what not to do in the future they do not shed light on what is available to 'turn things around' now that you're here. The use of Radio frequency to stimulate collagen and elastin has launched a new wave of devices used to tighten skin and underlying superficial muscles (and in some case remove/shrink fat).  We have used #Fractora for neck tightening with good success. Many of us have had sleepless nights because of a dreaded <em>toothache</em>.  The lucky ones may of had an inkling of warning, but most are totally caught off guard by this <strong><a href="http://www.dentistryofnaples.com/patient-services/emergency-care/" target="_blank">dental emergency</a></strong>.  As a practicing <strong><a href="https://www.blogger.com/"><span id="goog_383113199"></span>dentist in Naples, Florida<span id="goog_383113200"></span></a></strong>, there are some tricks that you can use to improve your comfort.<br />

Most often, when a patient experiences a toothache  it is associated with an abscessed tooth.  When a tooth abscesses, the nerve inside the tooth dies and becomes infected.  The resulting infection radiates pain throughout the jaw.  Very often, there may be little warning that such an infection is occurring.  Signs of a dental abscess include tenderness to chewing, throbbing pain, and the tooth may even loosen up.  Unfortunately,  intervention by your dentist  is necessary when a tooth becomes abscessed.  If the tooth is restorable, a root canal may be recommended, and if it's considered non-restorable, extraction of the tooth may be recommended.  Over-the-counter pain relievers  usually have minimal effect on a true dental abscess.  The best choice of action  would be to contact your dental office for an immediate evaluation.  An antibiotic may be prescribed which will usually relieve the discomfort  within a day.  Stronger pain medication may also be prescribed  as needed. <br />
<br />
The second-most common cause of a dental toothache would be a gingival abscess, or a "gum"  infection  around the tooth.  These types of infections exist on the outside of the tooth, and are usually caused by poor hygiene, history of periodontal disease, or quite possibly a foreign body, such as food  which gets caught under the gum line.  These types of infections have nothing to do with the nerve inside the tooth, and can be treated differently.   Many patients would try vigorous saltwater rinsing, mouthwash rinses,  and tooth brushing to help combat  this type of infection.  Normally, the gum is swollen around the tooth making it quite tender.  If it is in fact a foreign body causing the infection, much like a splinter in a finger, eventually the infection will pass once the foreign body is loosened. This typically would take a few days.  It is recommended that one seek  professional  advice from  a dentist  to verify  the cause of the infection.   Normally in the dental office,  the area would be cleansed, and any foreign debris or plaque removed.   The tissue would normally return to normal  within the week. <br />
<br />
 Lastly the most common toothache we see has dentists involves the fracture of the tooth.  If the fracture is significant,  it exposes the sensitive layer of the tooth  below the enamel,  and in extreme cases exposes the nerve.  When this occurs, dental intervention is absolutely necessary.  If the nerve has been  involved,  a <u>root canal</u> or <u>extraction</u>  be recommended.  If the fracture does not extend into the nerve,  the tooth may require restorations such as a filling,  or crown,  depending on the size  of the  broken area. <br />
<br />
As a practicing dentist,  I do recommend  that whenever people experience a dental toothache,  that he or she seek consult from a dentist  so that the proper diagnosis and treatment  can be offered.    Such a plan puts the patient's  health  and comfort  in top priority.<br />

For over a century, dental patients who are missing most, or

factors could be responsible for them being in this situation, however, the end

Most
denture wearers complain of discomfort,<span style="mso-spacerun: yes;"> 
</span>difficulty chewing, eating, and speaking because the dentures are not
permanently secured to the mouth.<span style="mso-spacerun: yes;">  </span>Some
<a href="http://www.dentistryofnaples.com/" target="_blank">dentures</a> may experience a certain level of “suction,” however, most dentures
can be removed with minimal effort by the patient.<span style="mso-spacerun: yes;">  </span>When dentures move around during a meal, food
particles and debris is allowed to seep <span style="mso-spacerun: yes;"> </span>underneath the plastic, and irritate the gun
tissue.<span style="mso-spacerun: yes;">  </span><br />

This leads to sore spots under
the appliance, which in extreme situations, may force the patient to forego
wearing the appliance all together.<span style="mso-spacerun: yes;">  </span>When
dentures move under biting forces, which includes all dentures, the underlying
bone is negatively affected, causing it to recede.<span style="mso-spacerun: yes;">  </span>Over time, recession can lead to even worse
sore spots, looser dentures, and even more food impaction.<span style="mso-spacerun: yes;">  </span>Fortunately, today there is a solution to all
these problems.<span style="mso-spacerun: yes;">  </span>It involves dental
implants.</div>

Traditionally,<strong> <a href="http://www.dentistryofnaples.com/patient-services/implant-dentistry/" target="_blank">dental implants</a></strong> have been used to replace one
or two missing teeth in and around other healthy teeth.<span style="mso-spacerun: yes;">  </span>Porcelain crowns would be fabricated, and
fixed to the implant to complete the smile.<span style="mso-spacerun: yes;">  </span><br />
<br />
Dental implants are not only for<em> <a href="http://www.dentistryofnaples.com/patient-services/crown-bridges/" target="_blank">porcelain crowns</a></em>.<span style="mso-spacerun: yes;">  </span>Routinely, today we use dental implants to
secure dentures in the mouth.<span style="mso-spacerun: yes;">  </span>Both
complete and partial dentures are able to accept dental implants to increase
the retention of the appliance.<span style="mso-spacerun: yes;">  </span>By
placing dental implants below a denture, it holds the appliance down firmly
using rubber gaskets and “o” rings, which allow the denture to snap into
place.<span style="mso-spacerun: yes;">  </span>The retention of the gaskets can
be modified to allow a custom fit which the patient will be comfortable with.<span style="mso-spacerun: yes;">  </span>When implants are utilized, the dentures are
strongly secured to the gun line, allowing less movement, less sore spots, and
much less food impaction below.<span style="mso-spacerun: yes;">  </span></div>

Most patients who wear dentures see the benefit of having

review of the patient’s medical history is a must.<span style="mso-spacerun: yes;">  </span>We are looking to confirm there is no
uncontrolled bodily disease, such as uncontrolled diabetes and hypertension, as
well as confirming the patient is healthy enough for surgery.<span style="mso-spacerun: yes;">  </span>A 3D <strong>dental CAT scan</strong> can be done in the
office to verify the presence of adequate bone thickness, and height where the
implants are planned.<span style="mso-spacerun: yes;">  </span><br />
<br />
Surgery usually
only lasts about an hour, and the patients don’t usually complain of any
discomfort after the procedure is done.<span style="mso-spacerun: yes;"> 
</span>Once the implants fully integrate into the bone, which takes 3-4 months
usually, the dentures can be secured to the implants.<span style="mso-spacerun: yes;">  </span>For most, this day is a life changing event

need to denture glues, leads to a more comfortable, secure, confident dental

Although the idea that we can tighten skin through the use of technology he is getting very close,  results have not matched the biochemical and biopsy results seen in treated skin.  I was very excited 190 saw what I believe was skin tightening with the use of lasers in 1995. <br />
<br />
This was the aggressive use of carbon dioxide based lasers.  We later found  that it really did not  tighten the skin, it did remodel the skin, sometimes with positive results.  Some of the other anergy based skin tightening systems are based on heating the collagen with radiofrequency and restoring some of the last in fibers. Sometimes clinical results are significant but they don't often correlate with the biopsy or biochemical results  And vice versa.<br />

A recent roundtable discussion involving some of the whirls experts in nonsurgical skin tightening and lasers discussed this very subject and seemed to put it in perspective:<br />

I have found  a more effective technology using radiofrequency with needleless so that the energy is transmitted into varying depths of the scan and even chest below the skin if we want to tighten and affect the superficial musculature such as the neck area or the lower lids.<br />

This technology is call <a href="http://dr-pena.blogspot.com/2014/04/am-i-good-candidate-for-fractora.html">Fractora</a>. the use of microneedling devices which stimulate in a similar fashion using skin puncture trauma to varying depths of the Skin can be augmented with the application of autologous platelet rich plasma.<br />

We have found that using the <i><b><a href="http://www.naples-md.com/2014/03/dermapen-needling-with-platelet-rich.html">Dermapen </a></b></i>device  in conjunction with autologous platelet gel, has given us tremendous clinical results albeit I have not biopsied the Streetman areas<br />
<br />
I am sure that we will continue our search for nonsurgical skin tightening as well as fat reduction and he will continue to yield a small but measurable results over the next few years.<br />

Over the last several years, dental researchers have been trying to develop an esthetic dental crown with all the great qualities of traditional gold crowns. They were looking to develop  white crowns with the same strength and predictability, same or improved fit, and same cleans ability and surface finish as gold crowns. What they developed was <strong><a href="http://www.dentistryofnaples.com/patient-services/crown-bridges/" target="_blank">Zirconium Crowns</a></strong>, and today, dentists around the world now use zirconium crowns in their offices daily.  <br />
<br />
Zirconium is a naturally forming element  found on the Periodic Table of Elements, with the symbol Zr.  When you look on this table, you'll find zirconium aligned with the metals. This is because it's characteristics mimic that of metal, however zirconium can be made white.  Traditionally patients  who grind their teeth, a condition known as bruxism, were offered gold crowns to restore molar teeth  because  routine porcelain crowns would fracture  under the forces of the bite. Today we can now offer the same patients a white crown alternative. <br />
<br />
Recently this zirconia crown underwent a three-year clinical performance study by the CRA foundation.  What they concluded has become a  total game changer in dentistry. Compared to other white crowns available on the market today, the zirconia crowns were found to be three times stronger, higher flexural strength, aesthetically pleasing, and most importantly, allowed for minimal preparation of the tooth. Since it requires minimal preparation, one could assume patients will  experience  much less  post-op sensitivity, less need for root canal therapy, and less fractured teeth. The foundation also concluded  that  these new crowns  are kind to the opposing dentition,  and because they are  created  using lasers and CAD/CAM technology, their fit is second to none.<br />

Once the dimensions are scanned and saved, the dental technician then creates the new crown on the computer using <em>CAD/CAM</em> software. This eliminates much of distortions of previous  dental crowns. Once it's designed on the computer, it sent to a 3-D milling machine where the crown is fabricated out of a solid block of zirconia .  Technicians can  match  the zirconium  block  to the color prescribed  by the dentist. Once milled,  zirconium can be polished  to a glassy shine  resembling  natural enamel.  Because of their strength and aesthetics,  they can be routinely used on natural teeth  as well as on dental implants. <br />

As a practicing <strong><a href="http://www.dentistryofnaples.com/" target="_blank">dentist in Naples Florida</a></strong>,  many of my patients  request  <em>tooth colored fillings</em> and crowns .  For my patients with a history of <u>bruxism</u>,  I routinely use zirconium  as the restorative material because of the strength and predictability.  It is my opinion, that it won't be too long before  metal  is completely  replaced  in dentistry  by more aesthetics  alternatives such as zirconium.<br />

Everyone wants to be a cosmetic physician. Either as a surgeon or as a cosmetic "medicine" provider. Regenerative medicine is HOT!  This includes stem cells and platelet derived growth factors. A lot of great research involving adipose  (fat) derived  stem cells is being done but very little of this is directed towards cosmetic surgery/ medicine.  The misconception that <i><a href="http://dr-pena.blogspot.com/2014/03/dermapen-with-prp-keeps-getting-great.html">Platelet Rich Plasma </a></i>(and all the goodies that come with it) involves stem cells is unfortunate. The stem cells that are already in your tissue as well as other "active cells" however, are stimulated by the many STIMULATING GROWTH FACTORS in platelet rich plasma.<br />

As usual the University based clinicians want control of the research and will slow down or discredit studies performed by private practitioners. Case in point being <a href="http://www.dr-pena.com/breast-augmentation.html">FAT GRAFTS FOR BREAST AUGMENTATION</a>.  This was developed and perfected by private practitioners when an old "Guru" said this should not be done and sealed it's fate in the academic world. Another such paper was just published. This <b><i><a href="http://www.wfmj.com/story/26189202/most-cosmetic-procedures-based-on-stem-cells-are-bogus" target="_blank">article</a></i></b> correctly states the limited scientific studies "PROVING" stem cells beneficial in cosmetic procedures. This however, does not disprove that they may be helpful in aesthetic surgery/medicine. Intuitively, there should be a lot of potentials for the use of autologous adipose derived stem cells and platelet rich plasma in many forms of rejuveantion surgery. Case in point being the use of <b><i><a href="http://www.naples-md.com/2011/12/plastic-surgery-breast-implants-face.html">Fat Grafts for Facelifts</a></i></b>. The skin changes seen in the areas over the fat grafts cannot be explained by "filling effect" alone!<br />

<iframe allowfullscreen="" frameborder="0" height="315" src="//www.youtube.com/embed/5-nmyzrccbI?rel=0" width="100%"></iframe>The Oral Cancer Foundation recently reported that over 44,000 Americans will be diagnosed this year with <strong>oral cancer</strong>, and 8000 deaths can be expected.  Traditionally, oral cancers have not gotten the publicity and the media coverage of other common cancers such as breast, lung, and prostate cancer.  That's unfortunate, because the 5 year survival rate of a patient diagnosed with oral cancer stands at 50-60 percent.  We will examine these findings, and hopefully shed light on how best to prevent this aggressive cancer.<br />

Traditionally, oral cancers were predominantly seen in older patients with history of tobacco use, as well as in patients who regularly consumed alcohol.  Smokeless tobacco still plagues its users as a major causative agent, however a new player has taken over the top spot.  This player is the Human Papilloma Virus 16, or HPV16 for short.  This sexually transmitted virus not only is increasing the incidence of oral cancer, but also is the driving cause of cervical cancer today.  Changes in sexual behavior has been blamed for the recent increase in newly diagnosed cases.  <br />
<br />
Oral cancers are dangerous, and often underdiagnosed, because of several key facts surrounding the disease.  Most cancers occur in the back of the mouth, and extend into the throat, making them hard to find.  Most show little to no color change initially, which greatly increases the likelihood of metastasis, which is estimated to be around 20%.  Most lesions are absolutely painless, and can easily go undetected for some time.  <br />
<br />
Men are twice as likely to develop oral cancers compared to women, with the average age of initial diagnosis being around age 50.  Black Americans are affected twice as often as white Americans.  This cancer, which is squamous cell 90% of the time, is an equal opportunity offender.  Tobacco users, alcohol abusers, and patients with persistent HPV infection are at highest risk of development.  <br />

What are the warning signs everyone should look for?  Any white or red patch found in the mouth or on the lips which doesn't heal in 14 days ought to be checked by a dentist.  These lesions may or may not be ulcerated, and are often non painful.  Any lump or mass felt in and around the mouth must be checked.  Difficulty swallowing, or painful swallowing or chewing could also be a warning sign.  A dentist would be the best professional to see initially to aid in the diagnosis.<br />
<br />
Oral cancers have a dismal 50% 5 year survival rate according to statistics.  Patients are generally treated with surgery, radiation, and in some instances, chemotherapy.  With such a low survival rate, early prevention is the key.  As a <strong><a href="http://www.dentistryofnaples.com/" target="_blank">Naples Florida dentist</a></strong>, I treat a wide variety of patients.  I regularly perform<em> oral cancer screenings</em> on all new patients, and existing patients are examined each time they return for hygiene recare.  If we as a nation took better care of our bodies, cut back on tobacco and alcohol use, and practiced more responsible sexual practices, the incidence of oral cancer would dramatically be reduced.  Until then, visit your<u> dentist</u>, and stay proactive.<br />

Just the thought of having to see a dentist for a tooth extraction usually sends chills down ones' spine.  Traditionally, the dentist would numb the area, remove the tooth, and send you home biting on gauze, with a tap on the back.  History has shown us that this treatment has not been the best treatment that could be offered.  Research studies have concluded that 40-60% of the surrounding bone is also lost within the first 2 years following the tooth extraction.   Fortunately today, the dental patient has options available to them which can prevent, or significantly reduce the loss of bone following an extraction.<br />

The procedure to prevent bone loss is called "Socket Preservation," which is a fancy term for bone grafting.  Immediately following the extraction of the tooth, bone grafting material is gently placed in the resulting tooth socket, and filled to the top.  In many cases, a resorbable collagen membrane is placed over the site to prevent the bone material from trickling out prematurely.  By placing the bone graft in the socket, it acts as a scaffold for the bone growing cells, and growth factors to adhere.  Over an 8-12 week period, most of the bone graft would have been replaced by the bodies own new bone.  This results in a solid block of bone which can now be utilized for a <u><a href="http://www.dentistryofnaples.com/patient-services/implant-dentistry/" target="_blank">dental implant</a></u>, or to support a porcelain bridge or <u>denture</u>.<br />

When dental implants are desired, it is necessary not only to have sufficient height and width of bone, but also, the bone density is important.  By grafting an extraction site, the bone density of the grafted site usually exceeds the original bone.  This enhances the implants predictability, lifespan, and overall success rates.<br />

Many patients choose to replace teeth with a porcelain bridge.  Bone grafting becomes important because the grafted bone helps to support the teeth anchoring the bridge.  This improved support extends the longevity of the bridge, and the graft enhances the overall esthetics of the bridge by making the false tooth look more realistic.<br />
