It functions to protect you from the outside world while keeping your bodily fluids inside.  Many billions of dollars have been spent developing methods to deliver drug strew intact skin. Recently a whole family of devices have been developed at poke holes through this protective barrier to stimulate the bodies own collagen production. This treats conditions  like <i>photoaging </i>(sun damage), acne scarring, fine lines, scarring, and hyperpigmentation.  We have been using devices such as micro-needles, lasers, and<a href="http://www.dr-pena.com/la-piel-spa.html"> micro-dermabrasion</a> to increase collagen production mechanically by injuring the deeper layers.  For quite awhile basic nutrients and vitamins have been applied to the recently injured skin thinking that this may get into the deeper active layers of the skin and have more and affect.  It has been found out that the lines of communication to the deep "active" layers of the skin stay open from 20 to 45 minutes after these treatments.  The use of platelet rich plasma after micro-needling such as the <a href="https://www.youtube.com/watch?v=KMt2MyJwhD0">Dermapen procedure</a> has shown some promising results.<br />

I just returned from a cosmetic meeting in Vegas which featured experts from plastic surgery, facial plastic surgery, dermatology, and oculoplastic surgery.  While new surgical procedures are always exciting to see the most talked about hot topic was the delivery of drugs through the skin after fractional treatments. Netherly needles were used but also ablative lasers such as the Smartxide Dot CO2 fractional laser. The areas that were being treated included hypertrophic scars, hypopigmentation, alopecia, stretch marks, and precancerous and cancerous lesions of the skin. The agents that were being used range from triamcinolone  (a potent injectable steroid) and 5-FU ( and anti-cancer medication) for hypertrophic scars, retinoic acid and poly-L-lactic acid (Sculptra)for stretch marks, and finally bimatoprost (Latisse)  4 hypopigmentation and alopecia.  Studies performed with scars on wounded warriors have yielded tremendous results which have been incorporated in trials with berm patients.  Young women suffering from hormonally affected stretch marks can get significant improvement using some of these techniques in a relatively short time.

I believe the use of transdermal drugs delivery systems after fractional injuries it's in it's infancy.<br />

STANFORD, Calif. — A blood sample could one day be enough to diagnose many types of solid cancers, or to monitor the amount of cancer in a patient's body and responses to treatment. Previous versions of the approach, which relies on monitoring levels of tumor DNA circulating in the blood, have required cumbersome and time-consuming steps to customize it to each patient or have not been sufficiently sensitive.<br />

Now, researchers at the Stanford University School of Medicine have devised a way to quickly bring the technique to the clinic. Their approach, which should be broadly applicable to many types of cancers, is highly sensitive and specific. With it they were able to accurately identify about 50 percent of people in the study with stage-1 lung cancer and all patients whose cancers were more advanced.<br />
<br />
"We set out to develop a method that overcomes two major hurdles in the circulating tumor DNA field," said Maximilian Diehn, MD, PhD, assistant professor of radiation oncology. "First, the technique needs to be very sensitive to detect the very small amounts of tumor DNA present in the blood. Second, to be clinically useful it's necessary to have a test that works off the shelf for the majority of patients with a given cancer."<br />
<br />
The researchers describe their findings in a paper that will be published online April 6 in Nature Medicine. Diehn shares senior authorship with Ash Alizadeh, MD, PhD, assistant professor of medicine. Postdoctoral scholars Aaron Newman, PhD, and Scott Bratman, MD, PhD share lead authorship.<br />

"We're trying to develop a general method to detect and measure disease burden," said Alizadeh, a hematologist and oncologist. "Blood cancers like leukemias can be easier to monitor than solid tumors through ease of access to the blood. By developing a general method for monitoring circulating tumor DNA, we're in effect trying to transform solid tumors into liquid tumors that can be detected and tracked more easily."<br />
<br />
Even in the absence of treatment, cancer cells are continuously dividing and dying. As they die, they release DNA into the bloodstream, like tiny genetic messages in a bottle. Learning to read these messages — and to pick out the one in 1,000 or 10,000 that come from a cancer cell — can allow clinicians to quickly and noninvasively monitor the volume of tumor, a patient's response to therapy and even how the tumor mutations evolve over time in the face of treatment or other selective pressures.<br />
<br />
"The vast majority of circulating DNA is from normal, non-cancerous cells, even in patients with advanced cancer," Bratman said. "We needed a comprehensive strategy for isolating the circulating DNA from blood and detecting the rare, cancer-associated mutations. To boost the sensitivity of the technique, we optimized methods for extracting, processing and analyzing the DNA."<br />
<br />
The researchers' technique, which they have dubbed CAPP-Seq, for Cancer Personalized Profiling by deep Sequencing, is sensitive enough to detect just one molecule of tumor DNA in a sea of 10,000 healthy DNA molecules in the blood. Although the researchers focused on patients with non-small-cell lung cancer (which includes most lung cancers, including adenocarcinomas, squamous cell carcinoma and large cell carcinoma), the approach should be widely applicable to many different solid tumors throughout the body. It's also possible that it could one day be used not just to track the progress of a previously diagnosed patient, but also to screen healthy or at-risk populations for signs of trouble.<br />
<br />
Tumor DNA differs from normal DNA by virtue of mutations in the nucleotide sequence. Some of the mutations are thought to be cancer drivers, responsible for initiating the uncontrolled cell growth that is the hallmark of the disease. Others accumulate randomly during repeated cell division. These secondary mutations can sometimes confer resistance to therapy; even a few tumor cells with these types of mutations can expand rapidly in the face of seemingly successful treatment.<br />
<br />
"Cancer is a genetic disease," Alizadeh said. "But unlike Down syndrome, for example, which has a single dominant cause, for most cancers it's very difficult to identify any one particular genetic aberration or mutation that is found in every patient. Instead, each cancer tends to be genetically different from patient to patient, although sets of mutations can be shared among patients with a given cancer."<br />
<br />
So the researchers took a population-based approach. National databases such as The Cancer Genome Atlas contain DNA sequences of tumors collected from thousands of patients — and pinpoint places in which the cancer DNA differs from normal DNA. Although the significance of each individual change is not always clear, it's becoming possible to generate a mutational fingerprint for each cancer type that includes nucleotide changes, insertions or deletions of short pieces of genetic material and translocation events that shuffle or even flip DNA regions. Although no patient will have all the mutations, nearly all will have at least some.<br />
<br />
The group began by using a bioinformatics approach to collect information from the atlas on 407 patients with non-small-cell lung cancer, looking for regions in the genome enriched for cancer-associated mutations.<br />
<br />
"We looked for which genes are most commonly altered, and used computational approaches to identify what we call the genetic architecture of the cancer," Alizadeh said. "That allowed us to identify the part of the genome that would be best to identify and track the disease."<br />
<br />
They identified 139 genes that are recurrently mutated in non-small-cell lung cancer and that represent about 0.004 percent of the human genome. Next, the team designed oligonucleotides, panels of short pieces of DNA, bracketing these regions. The oligonucleotides were then used to perform very deep sequencing (meaning each region was sequenced about 10,000 times) of the surrounding DNA.<br />
<br />
"By sequencing only those regions of the genome that are highly enriched for cancer mutations, we're able to keep costs down and identify multiple mutations per patient," Diehn said.<br />
<br />
In contrast, other methods of tracking circulating tumor DNA have relied on single, well-known mutations that nevertheless are unlikely to occur in every patient with a particular cancer. Tracking more than one mutation increases the sensitivity of the approach and allows researchers more flexibility in seeing how the cancer changes over time.<br />
<br />
"There are currently no reliable biomarkers available for lung cancer patients, which is the most common cancer and No. 1 cause of cancer deaths," Diehn said. "We are very excited about our findings because a personalized, clinically useful biomarker could revolutionize how we detect and manage this devastating disease."<br />
<br />
Next, the researchers used these oligonucleotides to selectively sequence tumor samples from patients with the disease and identify specific mutations in each patient's tumor. Starting with a predefined panel of oligonucleotides allowed the researchers to quickly home in on patient-specific mutations that could be used to monitor disease.<br />
<br />
"A key advantage of our approach is that we can also track many different classes of mutations, and integrate information from all of them to get a much stronger signal," Newman said. "We've also developed statistical methods to suppress the background noise in a sample. This allows us to identify even very minute quantities of cancer DNA in a blood sample."<br />
<br />
When the researchers applied the technique to patients with non-small-cell lung cancer, they found they could detect disease in all patients with stage-2 or higher disease, and in half of those with stage-1, the earliest stage of disease. Furthermore, the absolute levels of circulating tumor DNA were highly correlated with tumor volume estimated by conventional imaging techniques such as CT and PET scans. This suggests CAPP-Seq could be used to monitor tumors at a fraction of the cost of commonly used imaging studies.<br />
<br />
CAPP-Seq may also be useful as a prognostic tool, the researchers found. The technique detected small levels of circulating tumor DNA in one patient thought to have been successfully treated for the disease; that patient experienced disease recurrence and ultimately died. Conversely, scans of a patient with early stage disease showed a mass thought to represent residual disease after treatment. However, CAPP-Seq detected no circulating tumor DNA, and the patient remained disease-free for the duration of the study.<br />
<br />
Finally, CAPP-Seq was also able to identify the presence in one patient of a minor population of tumor cells with a mutation that confers resistance to a drug commonly used to treat non-small-cell lung cancer.<br />
<br />
"If we can monitor the evolution of the tumor, and see the appearance of treatment-resistant subclones, we could potentially add or switch therapies to target these cells," Diehn said. "It's also possible we could use CAPP-Seq to identify subsets of early stage patients who could benefit most from additional treatment after surgery or radiation, such as chemotherapy or immunotherapy."<br />
<br />
The researchers are now working to design clinical trials to see whether CAPP-Seq can improve patient outcomes and decrease costs. They're also aiming to extend the technique to other types of tumors.<br />
<br />
Screening healthy but at-risk populations is another goal of the researchers. "It may be possible to develop assays that could simultaneously screen for multiple cancers," Diehn said. "This would include diseases such as breast, prostate, colorectal and lung cancer, for example."<br />
<br />
"This approach could, theoretically, work for any tumor," Alizadeh said. "We expect it to be broadly applicable across cancers."<br />

The most obvious sign of aging in the face is the loss of volume. Until recently, we would try to restore a youthful appearance by lifting the tissues that have the appearance of having fallen. We now know that a lot of that "fallen" appearance is actual <i><b><a href="http://youtu.be/duuQDHQoQ34" target="_blank">deflation. </a></b></i>Sure, there are factors such as the loss of elasticity and descent of tissues from gravity, however if only one of the factors is dealt with,  then you will never look like they did in the past. I use old photographs of the individual that I am considering for rejuvenation to determine what has changed so I can make the correct surgical and nonsurgical approach.  I used to like to start with photographs from high school her college graduation or perhaps a wedding photo and then see how the face progresses into the 30s and 40s. The majority of my patients come in to my office for facial aging in the 50s or 60s. I have seen quite a few patients take a proactive approach and get an opinion on what to do in their late 30s and early 40s. Sometimes in the individual's did not like there facial shape when they were young and liked the more sculpted look they developed in their early adulthood years. With the help of <b><a href="http://www.dr-pena.com/vectra-3d-imaging.html" target="_blank">Vectra 3D-( three dimensional photographic) system </a></b>we can analyze exactly what has taken place as the face changes over the years and make the appropriate changes.<br />
<br />
Using this technology I can map out the plan for restoring the desired shape and appearance to the face. The first building block is to restore volume in the critical areas such as the cheeks, the jaw line, and the brow and temporal area. Other areas of concern include the area around the eye especially the hollow of  the upper eyelid and the junction between the eyelid bag and the cheek (tear trough).<br />
<br />
Too many times patients feel their lips are too long. This sometimes is true but most often the problem is that the lip  loses volume and needs to get resculpted. In the lip it is particularly important to look at what the original lip volume was as well as the shape.  If one tries to change naturally small lips into <u><b><a href="http://youtu.be/Ym1-f9Tv7MI" target="_blank">full lips </a></b></u>the results can be rather obnoxious. The lip in particular loses volumes in two areas: the first being the actual lip volume and where the pink vermilion is visible in the front view and secondly in the lip line where the normal skin and the pink vermilion interphase. It is that second area of the lip line which is critical to treat with volume before addressing the overall shape of the lip. This lip line filling has a benefit of erasing fine lines that are often blamed on "smoking" or drinking through a straw. The volume loss in this area combined with loss of elasticity results in the spine lines which are often more visible when you put on lipstick and the lipstick goes up into these crevices. The 10X mirrors that are often used to apply makeup often exaggerate these lines and add to the magnitude of the concerns.<br />
<br />
 The cheeks and the areas just below it are often areas that are misunderstood by plastic surgeons and dermatologists.  The problem is that they  have a tendency to put too much volume into the cheeks in an unflattering exaggerated way. This is often made worse by the over stuffing of the lips creating a caricature of youth rather than restoring the youthful "natural appearance". Another area that loses volume and can easily be treated is the jaw line. Volume placed in front of the "jowl" and near the angle of the mandible can re-create the beautiful jawline and strengthen the chin that will set off the contrast between the face and the neck.<br />
<br />
Restoring volume to the orbital tissues ( eyelids) requires more study and more artistry. The tissues here are thin and a little bit goes a long way.  The temporal hollows can be restored to their natural youthful appearance with a little volume especially in the deep layers. The brow pad restoration can actually give the appearance of lifting the eyebrow.<br />
<br />
My favorite volume restoration has been <b><a href="http://youtu.be/HWSqY83k1vU" target="_blank">fat grafting</a></b> for many years. <i>Hyalouronic acid fillers such as Juvederm and </i><b><i><a href="http://dr-pena.blogspot.com/2014/01/voluma-in-naples-florida.html" target="_blank">Voluma</a></i></b> however are getting better and lasting longer. They are easily obtained, injected, and adjusted and even erased if needed.<br />
<br />
Sculptra is a collagen volume stimulator they can also be used in certain areas of the face to restore volume.  Under no circumstances should you let anyone inject silicone into your face or any other part of your body as the body's reaction can be quite unpredictable.<br />

Over the past 10 years, the popularity of <u>Dental Implants</u> has absolutely taken off.  There are many reasons for this, however, I believe the main reason lies in their versatility and predictability.<br />
<br />
Dental implants are titanium tooth replacements which are placed into the jaw.  They are very similar in shape and size to the actual tooth roots they replace.  The procedure to place them is surprisingly comfortable, with the use of only local anesthesia, and very often the whole procedure takes about as long as a typical filling.  The advent of  Dental CAT scanners has taken much of the guesswork out of the procedure, allowing the dentist to plan the surgery virtually beforehand.  This translates into a much safer, faster, and more accurate outcome.  There are 3 major uses for dental implants in todays' world.  <br />

Typically, Dental Implants are used to anchor either crowns or bridges securely into the moth.  They mimic the natural architecture of the dentition, they are color matched to be in harmony with the other teeth, but most importantly, they are the strongest tooth replacement option available.  They are not subject to decay, root canals, or periodontal disease.  Patients choose this option over others also because the teeth are secured to the implants permanently, and they don't come out.<br />

<u><a href="http://www.dentistryofnaples.com/patient-services/implant-dentistry/" target="_blank">Dental Implants</a></u> also can be used to anchor a fixed hybrid dental appliance.  I know that that's a big word, but its pretty easy to visualize.  A fixed hybrid appliance is like a denture, except instead of removing the appliance after meals or at night, the appliance is "fixed" or attached to the implants with special screws that only a dentist could remove.  This option is often chosen when a patient is missing most or all of the teeth in an arch.  It offers the patient the confidence that the teeth will not slide around of flop out while eating and speaking.  And because it's fixed with screws, and not cement, it can be removed, if necessary, without damaging  it.  Typically, these appliances have been fabricated out of plastic, like a denture, but recently,  porcelain hybrids are available which offer greater strength, wear resistance, and increased aesthetics.<br />

Lastly, Dental Implants are commonly used to secure complete and partial <u>dentures</u> in place.  Unlike the hybrid appliance, these implant dentures can be removed by the patient daily.  This allows the patients to very effectively clean not only the denture, but also the implants.  Special attachments, similar to "o" rings are placed into the denture, and these rings grab the top of the implants, and snap in.  This option is very popular because it requires only a limited number of implants to be placed, and allows patients to have the confidence of chewing and speaking without the teeth slipping.  This is a great service we offer for those suffering with loose fitting dentures.  Most patients who receive this treatment wish they would have done it years ago.  <br />

One of the most frustrating surgical endeavors that plastic surgeons undertake is the "Large volume breast lift". Either a large breasted patient wants to keep a lot of volume while lifting her breasts, or someone with saggy breasts and inelastic skin wants to be lifted and enlarged to a big size, Both cannot be dissuaded from going down the path of  inevitable frustration.<br />

The plastic surgeons (myself included) cannot convince the prospective surgical candidates of the wiser "moderate" size lifts.<br />
<br />
NOW we seem to have a solution that at least seems to give us long lasting (but probably not permanent) solution to <a href="http://www.dr-pena.com/breast-lift.html" target="_blank">BIG BREAST LIFTS</a>.<br />

For years the breast lift, augmentation has been the single most difficult common surgery that plastic surgeons perform. By creating two opposing forces (Lifting the breasts by shrinking the skin envelope and increasing the volume by adding an implant), we have put in motion a recipe for early relaxation and scars that may be less than optimal. There is a new material called <a href="http://www.dr-pena.com/breast-lift.html" target="_blank">SERI SCAFFOLD which is used in Breast Lift Mastopexy</a>.<br />
<br />
This is a fabric or mesh that is made from the silk threads. Turns out that silk is made from two proteins: one gives the fiber strength and the other makes it sticky. By removing the sticky protein and weaving the strength  protein a "natural' mesh support can be made that can be used to support your bodies tissues.<br />
<br />
The beauty of  SERI Scaffold is that it is eventually totally replaced by your body while maintaining it's strength! The material is replaced by a natural Type 1 (Good  collagen) 3 X the original thickness of the material. This natural collagen sheet remains for years! It has been used very successfully to maintain the shape of these large breast lifts for up to three years (and the results are still holding with very little stretch!).<br />
<br />
This has been used for repairing hernias as well as many other uses. The cost of the material is approximately $1000 per breast and that is inexpensive compared  to the cadaver skin and porcine and bovine products available for similar applications.<br />

<span class="s1">Before the advent of <a href="https://plus.google.com/105567995968562327941/about?hl=en" target="_blank">dental implants</a>, patients who faced the reality of losing a tooth were told one of three possible solutions.  The patient could have the dentist fabricate a porcelain bridge, manufacture a plastic removable partial denture, or simply leave the space vacant. </span></div>

<span class="s1">When a porcelain bridge is fabricated, the adjacent teeth to the missing tooth are prepared, and shaved down so they could anchor the bridge.  A dental bridge traditionally has been the best option for replacing a tooth because it’s cemented into place, feels like normal teeth, can be matched to existing teeth, and only requires 2 appointments.  There are downsides to bridgework however.  Sometimes the adjacent teeth are healthy, pristine teeth.  Bridgework requires these teeth be shaved down unnecessarily.  Because these anchor teeth are natural teeth, they are subject to tooth aches, and cavities.  If a bridge gets a cavity below it, it usually means the bridge would need to be replaced.  Flossing around bridges can be tricky because all the teeth are attached, which could lead to possible cavities or periodontal problems.  </span></div>

<span class="s1">Plastic partial dentures can be fabricated as well to replace a missing tooth.  Partials differ from implants and porcelain bridges because they are not cemented in, they are removable appliances.  Although this could aid in cleaning the appliance, it poses several challenges to the patient.  Eating is most challenging, due to the fact that the partial moves when pressure is applied.  This could cause soreness in the gums.  Food also seems to get under even the best fitting dentures.  The major advantage dentures offer is financial.  They are the most cost effective solution for tooth replacement.  But are they worth the aggravation?  That’s left up to the patients’ to decide.</span></div>

The tour  begins as you enter the front door. While you wait for your appointment you will relax in our beautifully appointed waiting room. You may opt to glance over a magazine  or examine our breath taking salt water aquarium.<br />

If  You had an appointment with an aesthetician, massage therapist, pedicurist, or our physician's assistant at La Piel Spa... you will then enter the door on your right. You will be greeted by the receptionist at the spa desk and directed to one the treatment rooms where you will meet your therapist or physician's asssistant.<br />

If your appointment is with Dr. Pena... you will be escorted into the door on the left chest beside the reception desk. If the appointment is for the consultation or a "view photo" view be entering Dr. Pena's office.  If you're here for a postoperative visit or some other concern he will be placed in one of the exam rooms.  Photographs including the 3-D Vectra images of the face and breast and body will be taken in the photo room to help Dr. Pena properly plan all the surgical and nonsurgical options. Your old photographs will be extremely helpful for this exercise.<br />

Postoperative patient's and patient's entering on the day of their schedule surgery will enter through the surgical drive-through door. This insures the ultimate in privacy for our surgical patients. Here you will be escorted into the preoperative room where you will be prepared for the upcoming procedure. If anesthetic he has been scheduled, you will meet and discuss this with the anesthetist as they formulate your anesthesia plan. After conferring with Dr. Pena in the final markings are placed, you are escorted into the operating suite. Dr. Pena has designed a state-of-the-art operatory with a giant window leading to a private garden. This scenario allows the patient to relax in this environment. This facility is accredited by AAAASF (American Association for Accreditation of Ambulatory Surgical Facilities) and Dr pena is a member of the American Society of Plastic Surgeons. The ASPS is comprised solely of Board Certified Plastic Surgeons.<br />
<br />
After your procedure you will be taken to the recovery room where you will be carefully watched as you fully awaken. When this has been accomplished the individual you have appointed to take care of your during the first postoperative day will be asked to come in and discuss the instructions and precautions. You will then exit via the perioperative door. This is the same door you will use the next day when you come for your first postoperative day visit.<br />

The use of three dimensional camera and software allows me to perform surgery and view the results in front of the patient BEFORE the actual surgery. If either of us do not like what we see we change the plan! This has been extremely helpful in deciding the breasts implant size as well as how much and where to place the volume in facial aesthetics.  Chin Augmentations and Rhinoplasty have also been simulated in 3-D to aid in discussions and planning.<br />

Some patients are overwhelmed by the details and seem to "freeze up" worried that they might make the "wrong" decision by increasing or decreasing their breast implants by as little as 300 ccs (1 ounce)!. I am seeing these patients  multiple times and I fear that they think that what they are seeing is a warranty of results and that their individual tissues and their own healing properties have nothing to do with the actual results! <u><b>Botox is not just for the upper face anymore</b></u>. While the majority of uses for <i>Botox</i> is around the eyes I find it extremely helpful to treat many conditions which bother my patients in Naples. I have used <a href="http://youtube.com/watch?v=-mxsirCPFKs" target="_blank">Botox for chin dimples</a> and <a href="http://youtube.com/watch?v=-mxsirCPFKs" target="_blank">Botox for smokers lines</a> also known as the <a href="http://youtube.com/watch?v=-mxsirCPFKs" target="_blank">smoker lines treatment</a> without fillers.  Very small amounts of <i>Botox are used around the mouth to prevent asymmetries and dysfunction of the lips</i>.  They can be injected very superficially to treat fine lines which are to maintain smokers lines but can often be a sign of pure inelastic skin caused by her attitude, sun damage, and/or smoking.  Some individuals with very expressive faces or a significant overbite will keep the lower lip tightened to close the lips and prevent the open gaping teeth look.  When I treat chin dimples with Botox I tell the patient's that this is a subconscious act almost like an expression and we can train their bodies to minimize this deformity even after the Botox wears off. The other use for Botox around the mouth is to elevate the corners of the mouth in those individuals that because of their expressions have down turned mouth giving a sad appearance. The root of the muscle which causes this is at the junction of the jaw line and chin and weakening these muscles help neutralize or elevate the corners of the mouth given a pleasing appearance.<br />

Not a day goes by that an established patient in the practice, or a new patient, calls <b>complaining of a tooth ache</b>.  A typical toothache is described as a dull, throbbing pain, which radiates through the tooth, and up into the patient’s face and head.  Very often, patient’s try home remedies such as ice compresses, salt water rinses, and over the counter medications.  The problem with those remedies is that they only soothe the symptoms, not the problem.  Only a dentist can help you with a toothache.  Many patients experience temperature sensitivity with their teeth.  Extreme cold sensitivity could be a sign of a fractured tooth, cavity, or the beginning of  a tooth infection.  Sensitivity to hot liquids, or food, usually indicates a dying, or dead nerve in a tooth, which will need immediate attention.  Typically, toothaches can be treated by fixing the problem with the tooth, and restoring it  back to its original strength.  When a tooth becomes infected, simply taking antibiotics like you do for a sore throat simply will not work.  Without physically removing the source of the infection, tooth infections will reoccur.  When they do, they do with a vengeance.  In those cases, the infected nerve can be removed, the center of the tooth sterilized, and the tooth can be saved.  If the cavity or break is so severe that the tooth can’t be saved, an extraction would be performed to remove the source of the infection.<br />

<i><a href="http://www.dentistryofnaples.com/" target="_blank">Emergency Dental Trauma</a></i> to the mouth is usually to blame.  Car accidents, sports injuries, slip and fall accidents contribute to many of the broken teeth we see.  But perhaps the biggest source of broken teeth we treat is the teeth themselves.  Many patients have teeth that are heavily restored with filling materials, which could actually weaken the tooth.  These teeth are vulnerable to fracture, and highly unpredictable.  The good news is that this is preventable. Weak, highly restored, teeth can<br />
be made stronger, and fortified with a porcelain crown.  The crown will distribute the biting forces over the entire tooth, and acts a jacket ove the tooth, holding it together, preventing fracture.<br />

Non surgical skin tightening and rejuvenation has reached the next level. The technology is <i>Fractora bipolar radio frequency  fractional skin rejuvenation</i>.  With minimal disruption of the superficial skin (which has very little to do with the quality of the skin) the Fractora Naples device delivers energy through many tiny needle to the deeper collagen and elastin rich layer of the skin. These pins range from 0.9mm to 3 mms in length and are pushed into the skin to deliver the energy. Some of the pins coated to allow the energy to bypass totally the surface of the skin and concentrate on the deep layers. The 3mm needles can tighten superficial muscles to some extent by stimulating/ heating them as the energy returns to the return pads.Because the superficial skin is minimally affected the recovery period is also minimized. usually swelling for a few days and resumption of activities within the week. The final result also will be delayed by 4-6 weeks after each treatment. The  Fractora treatment can be one "deep" treatment requiring an anesthetic (either local or intravenous sedation) or multiple (4-5) requiring only a topical cream anesthetic.<br />

Fat Graft Facelifts.<br />
<br />
For many years our facial rejuvenations have been incomplete!. Yes we were able to tighten and lift and scrape and burn... but the patients never really looked like they did when they were younger. Yes they looked better, just not quite in the direction that they came from. In the mid 1990s Dr Sydney Coleman introduced a method of fat grafting to achieve restoration of facial volumes lost during natural aging. The method of grafting in a methodical "threading" method allowed for a good to excellent "take". The full face fat grafts achieved rejuvenation without "surgery" (or at least non-cutting surgery).<br />
<br />
The most significant volume loss is noted in the malar or cheek area, followed by the area in front of the jowl at the border of the chin. The temporal areas and lateral brow pad. Restoring these areas with volume generated by fat grafts will return the youthful contours to the face. The fact that fat is full of stem cells explains the added benefit to fat grafts, which is improvement of the overlying skin quality. This is especially  important in individuals with a history of acne with subsequent scarring or sun damage with resulting thinning and loss of of elasticity. Over the first six months a noticeable improvement in the skin quality is usually noted.  When I only use fat for rejuvenation (no lifting), I like the term "Fat Graft facelift". I usually add fat grafts to my face and neck lifts thus enhancing the short and long term results. A more natural result is obtained combining the fat graft with the facial-plasty since the original shape of the face is restored before the lax tissues are tightened and excess trimmed.<br />
<br />
Long term fat graft survival has been questioned by many of my colleagues that feel fat is quite temporary. I just saw my first full face fat graft patient (originally grafted in 1998) the other day.  The fat and overlying skin were just like 6 months after the original procedure! The face naturally ages by losing volume so I expect some loss to naturally occur with time. I have "re-grafted" several individuals at 1 year because they really liked the "full volume" look, but generally I regraft at 5-7 years. Often patients have come in expecting me to recommend another facelift and are thrilled with the result of the volume restoration with fat grafts. Compared to so-called "liquid facelifts" that often cost $6000-$10'000, I offer a more "permanent" solution for under $5000 using your own excess fat. Talk about recycling! This procedure was touted originally as the "Stem Cell Facelift", however as stem cell isolation/purification procedures become more available (and FDA approved), more definitive stem cell manipulations and indications will become commonplace.Stay tune as the future of Re-genrative Medicine is bright.  Let our bodies heal themselves!<br />
