Fat Graft Facelifts.

For many years our facial rejuvenations have been incomplete!. Yes we were able to tighten and lift and scrape and burn... but the patients never really looked like they did when they were younger. Yes they looked better, just not quite in the direction that they came from. In the mid 1990s Dr Sydney Coleman introduced a method of fat grafting to achieve restoration of facial volumes lost during natural aging. The method of grafting in a methodical "threading" method allowed for a good to excellent "take". The full face fat grafts achieved rejuvenation without "surgery" (or at least non-cutting surgery).

The most significant volume loss is noted in the malar or cheek area, followed by the area in front of the jowl at the border of the chin. The temporal areas and lateral brow pad. Restoring these areas with volume generated by fat grafts will return the youthful contours to the face. The fact that fat is full of stem cells explains the added benefit to fat grafts, which is improvement of the overlying skin quality. This is especially  important in individuals with a history of acne with subsequent scarring or sun damage with resulting thinning and loss of of elasticity. Over the first six months a noticeable improvement in the skin quality is usually noted.  When I only use fat for rejuvenation (no lifting), I like the term "Fat Graft facelift". I usually add fat grafts to my face and neck lifts thus enhancing the short and long term results. A more natural result is obtained combining the fat graft with the facial-plasty since the original shape of the face is restored before the lax tissues are tightened and excess trimmed.

Long term fat graft survival has been questioned by many of my colleagues that feel fat is quite temporary. I just saw my first full face fat graft patient (originally grafted in 1998) the other day.  The fat and overlying skin were just like 6 months after the original procedure! The face naturally ages by losing volume so I expect some loss to naturally occur with time. I have "re-grafted" several individuals at 1 year because they really liked the "full volume" look, but generally I regraft at 5-7 years. Often patients have come in expecting me to recommend another facelift and are thrilled with the result of the volume restoration with fat grafts. Compared to so-called "liquid facelifts" that often cost $6000-$10'000, I offer a more "permanent" solution for under $5000 using your own excess fat. Talk about recycling! This procedure was touted originally as the "Stem Cell Facelift", however as stem cell isolation/purification procedures become more available (and FDA approved), more definitive stem cell manipulations and indications will become commonplace.Stay tune as the future of Re-genrative Medicine is bright.  Let our bodies heal themselves!
